Transformation of geriatric residences

Background: Global population aging demands a transformation of traditional gerontological care models toward approaches that prioritize the dignity and autonomy of older adults. This work presents a proposed systematized intervention program to transform nursing homes toward the Person-Centered Integrated Care (PCIC) model. Objective: To propose and validate a structured methodology to evaluate organizational maturity of nursing homes regarding the PCIC model and design an action plan that facilitates the transition from traditional care models. Methodology: Proposed 18-month intervention program structured in seven sequential phases, including diagnostic evaluation through organizational maturity matrix in ten key areas of the PCIC model, gap analysis, and development of prioritized action plan. Conclusions: The proposed program provides a methodological framework adapted to the Latin American context that could significantly contribute to the field of institutional gerontology, offering practical tools for improving quality of care in nursing homes.
